Adult Dose
Delayed Release Tablets: 300mg BD for 2 doses and then 300mg once a day (for prophylaxis and therapy)
Oral suspension: 200mg thrice a day (for prophylaxis) and 400mg twice a day (for therapy)
IV formulations: 300mg BD for 2 doses and then 300mg once a day
Pediatric Dose
For pediatrics:
Oral Suspension: Oral suspension of posaconazole is recommended at 200mg thrice a day for 6months to 6 years and 300mg thrice a day for 7-16 years
IV formulation: 6mg/kg IV BD for2 doses and then 6mg/kg OD
